Mental Health Care in Washington DC Shouldn't Come Down to What You Can Afford
You searched for sliding scale therapy in Washington DC because something in your life is asking for attention. Maybe it is anxiety that won't quiet down. Maybe it is a relationship that keeps hitting the same wall. Maybe you are carrying the weight of immigration, identity, loss, or simply the exhaustion of being strong for everyone around you.
You are not alone. And you deserve help.
The cost of therapy in Washington DC can feel impossibly out of reach. Insurance coverage has gaps. Private pay rates at most therapists run well above what most working adults can afford week after week. Free mental health services and community mental health centers often prioritize people in acute mental health crisis, leaving a gap for those who need regular mental health support but are not in emergency situations. You end up in a cycle of wanting care and being priced out before you ever schedule an appointment. That gap between needing mental health services and affording them is real, and it is not your fault.
Healing Looks Different for Everyone.
Here's What It Felt Like for Them.
What Is Sliding Scale Therapy?
Sliding scale therapy is a pricing model where mental health providers adjust their fees based on a client's income, financial situation, or ability to pay. Instead of a flat rate, sliding scale fees create flexible options that make individual therapy accessible to a wider range of people across Washington DC and Maryland. It is one of the most common alternatives to insurance-based mental health care for people seeking affordable therapy in the area.
Most sliding scale programs ask clients to self-report their financial situation or choose a tier that honestly reflects where they are. It is built on trust. When more people can access mental health services and get the help they need, the whole community benefits. Sliding scale fees are not a workaround. They are a values statement about who deserves care and what mental health providers are willing to do to provide it.
How Does the Green Bottle Fee Model Work?
The Green Bottle Fee Model is the tiered pricing program developed at Life Migration Therapy. It is a community-centered approach to sliding scale fees rooted in economic justice and psychological safety. The idea is simple: those who are able to pay more help sustain access for clients who cannot. Every tier funds the next. This is how affordable therapy becomes something more than a marketing claim.
This is not a discount program. It is a values-based structure that acknowledges a basic reality: people in the same city, facing the same mental health conditions, are not all starting from the same financial place. The Green Bottle Fee Model was developed so that the cost of mental health treatment is never the barrier standing between you and the support you need.
You choose your tier. There is no income verification, no documentation required, and no judgment. The only ask is honesty. If your financial situation changes, your tier can change too. The therapists in this program are committed to making sure sliding scale fees work the way they are meant to: as a genuine tool for access, not a performance of affordability.

Who Is This Waitlist For?
This waitlist is open to adults 18 and older who live in Washington DC or Maryland. All therapy is delivered online, meaning you can access virtual mental health support from home, your office, or anywhere with a private internet connection. Online therapy provides the same quality of care as in-person work. It removes barriers of transportation and scheduling that often make it harder for busy people to get consistent help. Services are not available to residents of Virginia or other states at this time.
The communities this program is designed to serve include first- and second-generation immigrants navigating resettlement, identity, and belonging. Black, Brown, and Caribbean adults who want a therapist who understands cultural context without needing a history lesson. LGBTQ+ individuals looking for affirming, non-pathologizing care. Professionals and caregivers experiencing burnout. Adults managing anxiety, depression, grief, postpartum adjustment, relationship challenges, or the weight of living in a society that does not always make room for them.
If you are experiencing a mental health crisis or need emergency mental health services, this waitlist is not the right first step. Please reach out to the DC Department of Behavioral Health's Access HelpLine, or call or text 988 to find immediate mental health support through the Suicide and Crisis Lifeline. Those mental health resources exist to help you right now. This program is for ongoing, scheduled psychotherapy with adults who are ready for sustained care.

Who Provides Therapy Through This Program?
Therapy through the Green Bottle Fee Model is provided by associate clinicians working under the direct supervision of Charika White, LICSW, LCSW-C.
Charika is a licensed independent clinical social worker with licensure in both Washington DC and Maryland. She supervises all clinical work, reviews treatment approaches, and maintains active involvement in ensuring the quality of every client's care. All clinicians in this program are trained therapists operating within a supervised associate model, a recognized and established structure across mental health providers in the field.
This is not a university training clinic. The clinicians in this program are not doctoral students completing academic rotations or student clinicians fulfilling curriculum requirements. Many training clinic programs, including those affiliated with institutions like George Washington University, provide a valuable service to the Washington DC community. They operate on a different model: doctoral students and doctoral student clinicians rotate based on academic calendars, completing requirements for their psychology or social work programs. The associate clinicians at Life Migration Therapy work with greater continuity and within a focused, supervision-supported clinical framework.
That difference matters for the clients they serve.
Choosing a mental health provider is one of the most personal decisions you can make. Knowing who is trained to support you, who is supervising that work, and how your care is structured gives you the information you need to make that choice with confidence. Every person who joins this waitlist deserves to understand exactly what they are stepping into.
Why Is Affordable Therapy So Hard to Find in Washington DC?
Washington DC has no shortage of mental health providers. Clinics, group practices, psychology centers, and licensed therapists are listed across every directory. But low cost mental health services in Washington DC and Maryland tell a different story. Insurance coverage often excludes the therapists people most want to work with. Out-of-network fees put quality mental health care out of reach for most working people in the District. Free therapy programs and community mental health centers frequently serve people in active mental health crisis, leaving a gap for individuals who need ongoing support but are not in emergency situations.
The search for reduced fee therapy in Washington DC can feel defeating year after year. Waitlists at community clinics run long. Programs have narrow eligibility. Low cost services sometimes come with compromises in quality, continuity, or cultural fit. And when fees are the barrier, people delay getting help. Mental health treatment should not be something you have to wait to afford. The mental health conditions people are managing do not wait either.
Mental health care in Washington DC and Maryland should not be a luxury reserved for those who can afford top-tier private pay rates while everyone else searches through provider lists and comes up empty.
The Green Bottle Fee Model was developed to provide affordable therapy in DC and Maryland with the same quality, supervision, and cultural attentiveness that clients deserve at any price point.
What Mental Health Issues Does This Program Address?
The clinicians on this waitlist provide psychotherapy for a range of issues affecting adults across Washington DC and Maryland. This includes anxiety, depression, grief, identity and belonging, life transitions, relationship issues, workplace stress and burnout, postpartum adjustment, racial trauma, and the particular mental health conditions that arise from navigating immigrant life in the United States. It is designed for those ready to do the work of understanding themselves more deeply, managing what has felt unmanageable, and building a life with more room in it.
Treatment in this practice draws on evidence-based approaches adapted to the lived realities of the people it serves. An initial assessment helps your clinician understand your needs and develop a care direction that fits your goals. The focus is on building insight, developing sustainable coping skills, and finding calm in areas of life that have felt unstable. The aim is not just symptom management.
It is helping you connect with a version of yourself that feels more whole.
Frequently Asked Questions About Sliding Scale Therapy in Washington DC and Maryland
How much does sliding scale therapy cost in Washington DC?
Sliding scale fees at Life Migration Therapy range from $95 to $160 per appointment depending on the tier you choose. Tier 1, the Community Care Rate, is $95 per appointment for individuals experiencing financial hardship. Tier 2, the Sustainability Rate, is $130 per appointment for those who are financially stable but for whom full fees would create a strain. Tier 3, the Solidarity Rate, is $160 per appointment for individuals who can comfortably afford care. No income documentation or verification is required. You select the tier that honestly reflects your situation.
Is this the same as free therapy?
No. The Green Bottle Fee Model provides reduced fee therapy, not free therapy. Free therapy programs and community mental health centers exist across Washington DC and Maryland, but they often serve adults in active mental health crisis or carry long waitlists with limited availability. This program is designed to bridge the gap between fully private pay therapy and free services, making quality, culturally affirming psychotherapy affordable throughout the DC area.
How do I get started with sliding scale therapy in DC or Maryland?
The first step is completing the waitlist form on this page. You will be asked for your name, age, and state of residence. Once you are on the list, you will be contacted as openings become available. Joining the waitlist does not commit you to starting therapy. Complete the form and take it one step at a time. If you have questions before submitting, please email us directly and we will get back to you with the information you need.
Do you accept insurance for appointments through this program?
Life Migration Therapy does not bill insurance directly. Clients who choose the Solidarity Rate tier may request a superbill, a detailed receipt you can submit to your insurance provider for possible out-of-network reimbursement. Whether your insurance coverage includes out-of-network mental health benefits depends on your individual plan. Please call your insurance provider or check your member portal to learn about your out-of-network options before joining the waitlist. Many adults find that having detailed information about their insurance coverage helps them choose the right tier.
What is the difference between this program and a university training clinic?
University training programs, including those affiliated with George Washington University and similar psychology and social work programs across Washington DC, connect people with affordable therapy through doctoral students and doctoral student clinicians completing academic internships. These programs provide a real service to the community. The associate clinician model at Life Migration Therapy is different. Clinicians in this program are trained therapists in a supervised associate relationship with a licensed independent social worker. They are not rotating students fulfilling academic requirements. The supervision is ongoing and practice-based, not curriculum-driven. For clients seeking affordable therapy in DC, both options are worth knowing about. The right fit depends on what kind of continuity and framework matters most to you.
Who supervises the therapist I will be working with?
All clinicians in this program are supervised by Charika White, LICSW, LCSW-C, a licensed independent clinical social worker with licensure in Washington DC and Maryland. Supervision includes regular case review, treatment planning support, and ongoing clinical oversight. Clients in this program receive the benefit of their clinician's direct care alongside the experience of a trained and licensed clinical supervisor. The supervision structure is a feature of the model, not a footnote.
Is this program available if I am in a mental health crisis?
This program is not designed for crisis intervention or emergency mental health support. If you are experiencing a mental health crisis, please call or text 988 to reach the Suicide and Crisis Lifeline, or contact the DC Department of Behavioral Health Access HelpLine. Those mental health resources provide free, immediate support and can help you find crisis services across Washington DC and Maryland. This waitlist is for adults who are safe and ready for ongoing psychotherapy, not crisis intervention.
How long is the waitlist?
Waitlist availability changes based on clinician capacity. There is no set timeline to share. Completing the form below places you on the list and you will be contacted as openings become available. Joining does not commit you to anything. It is simply the first step.
Do you serve clients in Virginia?
Life Migration Therapy provides mental health services to adults in Washington DC and Maryland only. Virginia residents are not eligible for this program at this time. If you are in Virginia, we recommend searching for licensed therapists through Psychology Today, contacting your insurance provider for in-network referrals, or reaching out to the Virginia Department of Behavioral Health and Developmental Services for information about low cost services in your area.
Can couples or teens join this waitlist?
This waitlist is for individual therapy with adults 18 and older. It does not include couples therapy or services for teens and adolescents. Couples therapy and services for younger individuals may be available through Life Migration Therapy separately. Please reach out by email to connect with more information about those options.
What types of mental health issues do therapists in this program treat?
The therapists in this program are trained to support a range of mental health concerns affecting adults across Washington DC and Maryland. These include anxiety, depression, grief, relationship stress, burnout, postpartum adjustment, racial and identity-related trauma, and life transitions common to immigrants and first-generation professionals. During an initial assessment, your therapist will get to know you and the specific issues you are navigating. From there, a care approach is developed that fits your goals and your life. Psychotherapy through this program is not one-size-fits-all. It is built around the person sitting across from the clinician.
Is virtual therapy accessible if I have never tried it before?
Yes. Online therapy through this program is designed to be accessible to anyone with a secure internet connection, regardless of whether you have tried telehealth before. Your therapist will walk you through what to expect. Many clients find that virtual sessions feel surprisingly connected and calm, especially when the alternative is commuting to an office after a long day. If you have questions about the process or want to understand what getting started looks like, please reach out by email before joining the waitlist. We are happy to help you find your footing.
How do I know if the Green Bottle Fee Model is the right option for me?

You do not need a referral to join. You do not need to be a member of a specific community organization. You simply need to be ready to take a first step toward consistent mental health support.
